My 3-Year-Old Is Still Bottle Feeding

I have a 3-year-old baby girl, my only girl. I also have two older boys. I have been ready for the past two years to get rid of the bottle, but it just does not seem to happen. Every year on her birthday I say, "OK, tomorrow the bottle is gone." But another year goes by and she still has it. I have gotten it down to only a bedtime thing, cause for some reason she has to have her bear and her bottle. She even goes as far as to say, "Bababear!" She can't have one without the other. I would really love to have this stupid thing go away!!!
I tried buying her sippy cups that look like bottles but that doesn't work either. I even told her that the dog ate the bottle and I had to throw it away, but it seems like she always knows where there is another one. And it doesn't help that her father gives them to her earlier than he should!! I know that she should not have it anymore, especially because her teeth have become sooo bad from it, but I really don't know the right way or the best way to do it. If there is someone reading this with some good advice on ending bottle feeding, please help me!
