Problematic Preschooler

I have an almost 1-year-old girl and a 3 1/2-year-old boy. My son just doesn't listen. I will ask him a question like 10 times and he won't even look at me! I know he's not deaf (got him checked) and I know he's normal with brain development. He's had an MRI. For a while I thought it was because of him feeling ignored because he has a medically fragile sister who has brain tumors and epilepsy but, no, I don't think so.
I take him to therapy and out on special trips just him and me or his dad and him. I don't know what to do. It's getting to the point of not being safe. He'll start running, playing around, and keep going. And when we do get his attention he's so angry. He'll throw things and scream at us, and soil himself. He's not got a disability that is known. Any advice would be greatly welcome.
